Elizabeth meets standards in all categories of basic tenants. Good with communicating to patients on the role of occupational, will expand the scope to educate family members as well as staff.

Meets all standards for evaluations, will expand on completing the evaluation more independently. Good start with pulling information from the chart for the occupational profile. Will work on administrating standardized tests as well as creating appropriate goals for plan of care.

Very professional during working hours, she is also dependable as well as on time. Pt also asks questions and responds to constructive feedback. As well as shows flexibility. Improvement will be made for time management and volume of casework.
